TERMS AND CONDITIONS
(hereinafter, referred to as "Surety") to execute the bond herein applied for. The lndemnitor(s) agree that they are completing this Contract and Application honestly and voluntarily. The Following terms and conditions are an integral part of this Contract and Application dated:

and the lndemnitor and Surety agree that the issuance of said Appearance Bond is conditioned upon the full compliance of the following:
1. Surety shall have control and jurisdiction over the Defendant during the term for which the Appearance Bond is executed and shall have the right to approach, arrest, and surrender the Defendant to the proper authorities or officials as provided by law.
2. If the Defendant's case that is subject to this Contract remains on the docket of the Court past one (1) year from the execution of this Contract, an administrative fee of $150.00 per year shall be paid to Surety. An additional $150.00 per year shall be paid for each additional year.
3. In the event that the surrender of the Defendant is made prior to the Defendant's appearance in court and for reasons other than those enumerated in paragraph 4 below, the Defendant shall be entitled to a refund of the bond premium.
4. It is understood and agreed that the happening of any of the following events shall constitute a breach of the Defendant's obligations to Surety and Surety shall have the right to apprehend, arrest, and surrender the Defendant to the proper authorities or officials as provided by law and the Defendant and/or lndemnitor shall have no right to a refund of the bond premium whatsoever.
a. The Defendant departs or there is reasonable evidence to believe the Defendant is in the PROCESS OF DEPARTING the jurisdiction of the Court WITHOUT THE EXPRESS CONSENT of Surety.
b. The Defendant moves from one address to another without notifying the Surety either IN WRITING OR BY TELEPHONE.
c. The Defendant is ARRESTED, INCARCERATED, OR A WARRANT is issued for their arrest for any offense, other than a minor traffic violation.
d. The Defendant or lndemnitor(s) makes any false statement to Surety in this Contract and Application.
e. The Surety of Sureties WITHDRAW from their Surety-ship upon said bond.
f. The lndemnitor WITHDRAWS from their obligations under this Contract.
g. The lndemnitor or Defendant pays their premium fraudulently, i.e., counterfeit currency, unauthorized use of a credit or debit card, etc.
h. Any person on bond whose bond is over $1,000.00 must report to Surety by phone or in person each Monday of every week until bond is cleared on the docket of the Court in which it was issued.

The Undersigned hereby warrants that the forgoing declarations made and answers given are the truth without reservation and are made for the purpose of inducing the Surety to become surety or to procure suretyship on the bond or undertaking applied for herein, with the intent and purpose that they be relied on fully.
In addition, the undersigned hereby authorizes and directs his/her relatives, employers, bankers, the Federal Social Security Administration, the Internal Revenue Service, the State Department of Disability Insurance, the United States Armed Forces, the state Division of Motor Vehicles, all Municipal, County, State and Federal Law Enforcement Agencies, Surety and it’s assigns and/or duly authorized representative of the Defendant's apprehension for Court appearance, and for the purpose of securing reimbursement for any expenses incurred as a result of Defendant's non-appearance. The undersigned hereby waives his/her rights with respect to the Privacy Act and authorizes the use of copies of this document by Surety and its assigns and/or duly authorized representatives.
The undersigned authorizes Surety to run any necessary credit reports on my Social Security Number as it deems fit.
